All the eligible candidates are invited for Admission to MBBS/ BDS/ BAMS/ B.Pharm. Courses 2012

Eligibility Conditions

Passed Intermediate / 10+2 or equivalent examination in English, Physics, Chemistry and Biology (Zoology + Botany) taken together

For MBBS/ BDS/ BAMS

• A minimum of 50% marks for General and 45% for General-PC candidates and 40% marks for OBC#/SC/ST and respective PC candidates

For B.Pharm.

• A minimum of 60% marks for General / OBC and respective PC candidates and 55% marks for SC/ST and respective PC candidates. Candidates appearing in qualifying examination may also apply.

Required Age

Completed 17 years but not crossed 25 years as on 31st December, 2012 (Candidates born after 1^st Jan, 1996 and on or before 31^st Dec, 1987 need not apply).

How to Apply

• Sale of Application Form: 9^th Jan, 2012 to 5^th March, 2012 from the designated branches of Bank of Baroda as mentioned below:
• Application Form can be obtained from Bank of Baroda, BHU Branch, S.S. Hospital, Varanasi upto 12^th March, 2012
• Application Form can be obtained
• By Cash : Rs. 1500/- for GEN/OBC and Rs. 1000/- for SC/ST candidates from the following branches of Bank of Baroda.
  • Varanasi – (i) BHU - S S Hospital Branch, (ii) Ram Katora Branch, (iii) Orderly Bazar Branch
  • Delhi – (i) Parliament Street Branch, (ii) Nehru Place Branch, (iii) Chowk Shakti Nagar Branch
  • Kolkata – College Street Branch

  • Hyderabad – Main Branch, ABIDS Circle
  • Jaipur – DCM Branch

• By Post : From the Office of the Director (PMT-Cell), Institute of Medical Sciences, Banaras Hindu University, Varanasi-221005 by sending a written request through Registered Post along with Crossed Demand Draft/ Bankers Cheque (MICR only) of Rs. 1600/- for General/OBC candidates and Rs. 1100/- for SC/ST candidates of a Nationalized Bank, drawn in favour of “The Director, Institute of Medical Sciences, BHU” payable at Varanasi.
• Application form will be sent by Speed / Registered Post. However, Institute will not be responsible for any loss or postal delay.
• The request for application form must include the name and address of the candidate, and full particulars of the enclosed DD/BC (write candidate’s name and address on the back of DD). Two self-addressed, preferably typed, white paper slips of 10 cm X 6 cm size must also accompany it.
• The requisition for PMT Application Form should be super scribed “Request for Application Form for PMT-2012, BHU”. Last date of requisition for PMT form by post is 27^th Feb, 2012
• Application Forms from the Office of the Director (PMT-Cell), Institute of Medical Sciences, BHU, Varanasi-221005 can be obtained by POST only and not in person upto 27^th Feb, 2012.
• Application form duly completed must be sent through Registered Post so as to reach The Director (PMT CELL), Institute Of Medical Sciences, Banaras Hindu University, Varanasi-221005 latest by 15^th March, 2012

• PMT-2012 Screening: Wednesday, the 16^th May, 2012 at Varanasi, Delhi, Kolkata, and Hyderabad
• PMT-2012 Main: Sunday, 17^th June, 2012 at Varanasi Centre only
